We’re here to support children with complex disabilities and medical needs, working closely with families to create customised therapy plans that make a real difference. Our services are available to self-managed and plan-managed NDIS participants, ensuring families receive a flexible, responsive service.
Instructions for Completing a Referral
To complete the referral form, please provide as much relevant information as possible about the child’s current needs, medical history, and therapy goals. This helps us tailor our services and assign the right support team. After submission, we will review the referral and contact the family within a week to arrange an initial consultation or discuss any additional details needed. From there, we’ll initiate our therapy roadmap, ensuring each step aligns with the child’s and family’s needs. All information received will be kept private and confidential, and is only viewed by those who need it to establish a service.
